If you have successfully navigated the HCS Interest List and received your Medicaid waiver funding, congratulations! You have unlocked access to the most comprehensive disability support program in the state of Texas.
However, many families receive their funding and immediately ask: “So, what exactly does this pay for?” The HCS waiver is designed to cover almost every aspect of daily living, medical care, and community integration for individuals with intellectual and developmental disabilities (IDD). In this guide, we break down the complete list of HCS covered services into easy-to-understand categories so you can maximize your benefits.
Residential Support Services
The largest portion of HCS funding is typically dedicated to residential services. The state understands that every individual has different housing needs, so the waiver offers three distinct residential options:
1. Group Home Living (Residential Support)
For individuals who require 24-hour supervision and assistance, the HCS waiver fully funds placement in a 3-bed or 4-bed group home. These homes are staffed around the clock by trained direct support professionals (DSPs) who assist with bathing, cooking, medication administration, and transportation. Group home living is ideal for individuals seeking independence from their family home while maintaining a highly structured, safe environment.
2. Host Home / Companion Care
This option allows the individual with IDD to live in the private home of a trained caregiver (who is not their spouse or legal guardian). The caregiver provides daily assistance, supervision, and integration into the community. It offers a more intimate, family-like setting than a traditional group home.
3. Supported Home Living (SHL)
If the individual prefers to live in their own home or remain living with their parents/family, the waiver provides Supported Home Living. Under SHL, a caregiver comes to the family’s home for a set number of hours each week to assist with daily living activities, hygiene, and community outings.
Professional Therapies & Nursing
The HCS waiver provides extensive funding for professional medical and therapeutic services that standard Medicaid often limits. Your Service Planning Team (SPT) can allocate funds for any of the following:
- Nursing Services: Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) to administer medications, treat medical conditions, and oversee complex health needs.
- Physical Therapy (PT): To improve mobility, strength, and motor skills.
- Occupational Therapy (OT): To develop fine motor skills and the ability to perform daily tasks like dressing and eating.
- Speech and Language Pathology: To improve communication skills and assist with swallowing disorders.
- Dietary Services: Consultations with a registered dietician to ensure proper nutrition and manage health conditions like diabetes.
- Social Work Services: Support for the individual and family to navigate community resources and cope with the emotional aspects of disability.
- Cognitive Rehabilitation Therapy: Specialized therapy designed to help individuals recover or improve cognitive skills (memory, problem-solving, attention) following a brain injury or neurological condition.
Specialized Supports and Equipment
Beyond housing and therapy, the HCS waiver pays for the tangible items and modifications necessary for a safe, accessible life.
Adaptive Aids
Adaptive aids are medical equipment or devices that help an individual function independently. The HCS waiver can cover the cost of items that standard insurance refuses to pay for. Examples include:
- Specialized wheelchairs or positioning furniture
- Augmentative and Alternative Communication (AAC) devices (like speech-generating tablets)
- Specialized computer software
- Adult incontinence supplies (briefs, underpads)
- Sensory equipment
Minor Home Modifications
If the individual lives in their own home or their family’s home, the waiver provides a lifetime budget (currently up to $7,500, with additional repair allowances) to make the home accessible. Examples include:
- Installing wheelchair ramps
- Widening doorways for wheelchair access
- Installing roll-in showers or grab bars in bathrooms
- Modifying kitchen counters for accessibility
Dental Treatment
One of the most valuable, yet overlooked, HCS covered services is dental treatment. Standard Texas Medicaid provides very little dental coverage for adults. The HCS waiver provides a specific annual budget (up to $2,000 per year) for routine preventative care, fillings, extractions, and emergency dental work.
Day Habilitation & Employment
Integration into the community is a core philosophy of the HCS waiver. To support this, the waiver funds daytime activities and employment assistance.
Day Habilitation
Day Habilitation programs (often called “Day Habs”) provide a safe, engaging environment for individuals during standard working hours. Participants engage in social activities, educational games, life skills training, and community volunteer outings.
Supported Employment
If the individual wishes to work, the waiver pays for Supported Employment services. A job coach will help the individual build a resume, prepare for interviews, find a job in the community, and even accompany them to work to provide on-the-job training and support until they can work independently.
How to Maximize Your HCS Benefits
Receiving HCS funding is only the first step; to actually access these services, you need an exceptional provider.
Once a year, you will meet with your Service Coordinator and your provider to create a Person-Directed Plan (PDP). This document dictates exactly which of the services listed above you will receive. It is critical that you partner with an experienced, compassionate HCS provider who will advocate for your needs during this meeting.
