HCS Services

Service Coordinator

Each individual who is enrolled in the HCS has a service coordinator who:

Coordinates the development and implementation of the Individual Service Plan (ISP). The Individual Plan of Care is integrated services delivered and under the HCS program and through other sources.
Reviews rights-related, safety, and health issues.

Individual Socialization Skills Program
Our Individual Socialization Skills Program assists with day-to-day activities, and the individual will participate in recreational activities. Individual Socialization Skills Program is designed to assist them to acquire, retain, and improve independence, self-help, socialization, skills for daily living, and set personal goals.

Residential Services

We are offering a comfortable and secure environment for continuous living support. They are tailored to foster independence while providing necessary care—a home where personal growth and well-being are prioritized.

Frequently Asked Questions About HCS Services in Texas

What is the primary difference between HCS and regular Medicaid?
While standard Texas Medicaid primarily covers clinical medical needs like doctor visits, hospital stays, and prescriptions, the Home and Community-based Services (HCS) waiver pays for the daily supports needed to live safely in the community. This includes funding for housing (group homes or host homes), professional therapies, adaptive equipment, and daytime habilitation programs.

Do I have to choose a state-run facility if I get HCS funding?
No! The HCS program operates on a “Person-Directed” philosophy. Once the state approves your funding, you have the absolute right to interview and choose a private provider agency (like ANN&AGNES Healthcare Solutions) to actually deliver your care. You are never forced into a state-run institution.

Can you help if my child is transitioning out of high school?
Yes, this is one of the most critical times to secure HCS services. Transitioning out of the public school system often means losing access to daily structure and therapies. Our Day Habilitation and Supported Employment services are specifically designed to help young adults with IDD transition smoothly into active, fulfilling community life.

Will HCS pay to remodel my bathroom for wheelchair access?
Yes, under the “Minor Home Modifications” benefit, the HCS waiver provides a specific lifetime budget (currently up to $7,500) to make your private home safe and accessible. This can include roll-in showers, widened doorways, wheelchair ramps, and specialized grab bars.
